Mallon E. "Cook" Puckett, 65, passed away suddenly on Saturday, June 21, 2014. He is a native Middletonian.
He was preceded in death by his parents Hubert & Bertie Ann (Deaton) Puckett. He is survived by one daughter, Abbi Stafford; two sons, Brant Puckett & Jarred (Esselman) Puckett; three granddaughters; three sisters, Ladema Terrell, Valentina Fields & Catrina Tapp; two brothers, Ray & Tom Puckett; and numerous nephews & nieces.
A Celebration of Life will be held at Smith Park, Friday, June 27, 2014 5:00 pm – dusk.
