Thomas Robert Burk, 55, of Middletown, died Saturday, March 5, 2011, at his residence. He was born in Youngstown, Ohio on June 27, 1955.
Tom was a clerk at Jungle Jim's for 12 years.
Mr. Burk is survived by his wife, Annette (Andujar) Burk; daughter, Katherine Burk; son, Kevin Burk; mother, Susan Burk; three brothers, Charles (Linda) Burk, James (Michelle) Burk & Robert Burk; and three sisters, Janet Burk, Lisa (Dominic) Sebastian & Donna Farland.
He was preceded in death by his father, Robert E. Burk and sister, Linda Carol Salas.
Memorial service will be held Monday, March 14, 2011 at 5:30 pm at Berean Baptist Church, 6342 Hamilton Middletown Rd., Middletown with Reverend Wallace Marshall officiating. Family will receive friends prior to the service from 4:30 - 5:30 pm at the church.
